Longmont Department of Public Safety Case Management Services

Relationships rooted in trust lead to the best results. To that end, our collective harm-reduction / person-centered model prioritizes wellness and quality of life for those participating in our adult diversion program. We foster long-term relationships that explicitly avoid judgment or shame. Our specially trained peer case managers work for those struggling with substance use. Participants must be referred by a police officer, a hospital, or Medicaid partner. Alternatively, those seeking treatment may request to connect with a peer case manager.

Case managers assist with:

  • Connecting people to mental health care, substance use treatment and other support services
  • Navigate housing resources
  • Benefits enrollment
  • Employment support
  • Crisis management
  • Build relationship rooted in self-determination, acceptance and partnership.
