Tips for filing a successful and complete application
- Follow the City of Longmont’s application checklist to ensure you are submitting a complete application! Our checklist includes all state required documents plus additional city requirements.
- Make sure to include both City AND State fees with your application.
- If you pay your State fees online, provide a copy of your receipt to us with your application.
- The City only accepts business checks for payment of licensing and application fees.
- All application checklists are provided in English and Spanish. Some State forms, but not all, are available in Spanish and identified as SPANISH in the title. Only download the Spanish forms if you need them in Spanish. (updating in progress – if you do not find what you need, please give us a call!)

- Report of Changes (for change of trade name, duplicate copy of license, Retail warehouse storage permit)
- Retail Establishment (Same forms as Art Gallery)
- Special Event Permit (for alcohol service/sales)
- Tastings Permit
- Transfer of Ownership
