Liquor License Application Forms
Effective 7/1/2023 - The State Liquor Enforcement Division has increased their renewal fees by $125 effective July 1, 2023. The City of Longmont has updated our fee schedule to reflect this increase in state renewal fees.
Application Forms
The application forms are arranged below by application type. A comprehensive checklist is included in each folder, as well as all required forms.
- Use the City of Longmont's application checklist to ensure you are submitting a complete application
- All application checklists are provided in Spanish. Some forms, but not all, are available in Spanish and identified as SPANISH in the title. Only download the Spanish forms if you need them in Spanish.
- Consult the City's Liquor Licensing Fee Schedule (updated 7/7/23) to determine the fees required when you submit your application or renewal

Fingerprinting Information
You must be fingerprinted by one of the statewide fingerprinting vendors - Identogo or Colorado Fingerprinting. You can no longer go to the police department to be fingerprinted.
IdentoGo Fingerprinting Process
To schedule an appointment with IdentoGo, visit Local (City/County) Liquor Licensure
- When prompted, provide the City of Longmont's CBI Account Number for liquor licensing: CONCJ4357
- Provide payment
- For locations, please consult the their website
- You MUST have an appointment - DO NOT show up at any location and expect them to take your fingerprints
Colorado Fingerprinting Process
To schedule an appointment with Colorado Fingerprinting, visit www.coloradofingerprinting.com
- When prompted, provide the City of Longmont's codes: 4357LLQH
- Reason Fingerprinted : LIQUOR LIC - LOCAL PD SO 12-47-307
- You will be required to create an account. Please do so, follow the instructions, and provide payment with a credit card
- For locations, please consult their website or call 720-292-2722
- You MUST have an appointment - DO NOT show up at any location and expect them to take your fingerprints
