Community Health
Longmont Department of Public Safety's (LDPS) Community Health Program features a team of paramedics and a nurse practitioner who, through our partnership with Medicaid, serve community members experiencing chronic untreated or difficult to manage health issues due to any number of obstacles preventing them from connecting to regular medical care. Because of these barriers to well-being, a number of our residents are at an increased risk of needing emergency medical services and/or admission to local hospitals. The program aims to break down these barriers by conducting comprehensive assessments to look at physical and social needs, providing education on their health, and making connections to community resources. This is supported by a robust Case Management Team that specializes in addressing social determinants of health. As with every LDPS program, the goal is to bring both improved health and hope to those most in need of our shared support.
