Identity Theft Victim
Identity theft is the fastest growing crime in America, it is estimated that nearly 1 million people will be, or have been, a victim of identity theft.
In 2020-2021, many residents in Colorado were victims of an unemployment scam. If you were a victim of this unemployment scam, please fill out a online police report. You do not need to complete the documents below.
If You Are a Victim of Identity Theft
#1 Contact the law enforcement agency where the crime was committed or the law enforcement agency where you reside. A report will be taken and that law enforcement agency will investigate, per Colorado Revised Statute 16-5-103.

#2 Report the identity theft immediately to one of the credit reporting agencies and ask them to place a fraud alert on your account. The alert will remain on your report for about 90 days unless you choose to extend the alert or ask for a security freeze. If you call one agency, they will forward the fraud alert to the others:

#3 Fill out the ID Theft Affidavit above and sent credit dispute letters to companies for new accounts, or for fraud on existing accounts, and to the credit reporting bureaus.
#4 Consider putting a security freeze on your credit report. Information on security freezes can be obtained at the Boulder County District Attorney’s website or call for a copy of the information 303.441.3700.
#5 Close any affected accounts and stop payment on any stolen checks. Consider putting additional layers of security on any accounts that might be vulnerable.
#6 Do not use the same passwords and personal identification numbers that were on the compromised accounts – choose new ones.
#7 Notify the Department of Motor Vehicles if your driver’s license has been stolen.
#8 If your checkbook has been stolen notify your financial institution and check approval agencies:

#9 Notify the postal inspector and your carrier if mail is involved.
#10 Call the Social Security fraud hotline 1-800-269-0271 if you believe your Social Security information has been compromised.
#11 If an identity thief has used your personal information to create tax problems call the IRS at 1-877-777-4778 or go to the IRS website (www.irs.gov/advocate)
#12 For more information log onto the Federal Trade Commission (www.ftc.gov). Report Internet fraud to the Internet Crime Complaint Center ( http://www.ic3.gov/) or call FTC at 1-877-ID-THEFT (1-877-438-4338).
