Obtaining a report/record from ambulance, fire, police
Records Requested Pursuant to Colorado Open Records Act (CORA) and the Colorado Criminal Justice Records Act (CCJRA)
Because Longmont Public Safety is a criminal justice agency, most of its records fall under the Colorado Criminal Justice Records Act (CCJRA). As a result, records requests are generally governed by CCJRA rather than CORA. Criminal justice records related to an active case cannot be released until the investigation is closed, and release will not jeopardize any ongoing proceedings.
CORA’s recommended response times do not apply to CCJRA records. Except for “official action” records defined by statute, the release of criminal justice records is at the discretion of the records custodian, who may deny access if disclosure is determined to be contrary to the public interest.
All other records maintained by Longmont Public Safety can be obtained in two ways:
1. By mail.
A fee applies to all criminal justice records. Payment may be made by cash, check, or credit card. Credit card payments include a 2.95% processing fee.
Print and complete the English/Spanish request form (pdf), then mail it to: Longmont Department of Public Safety, Records Unit, 225 Kimbark Street, Longmont, CO 80501.
Please contact the Records Unit before mailing your form. Indicate whether you prefer to receive the report by postal mail or email.
2. In person.
Request forms are available at the Records Unit window on the first floor of the Safety and Justice Building, 225 Kimbark St., Longmont, Colorado. Staff will provide the cost of the requested document.
The records custodian will review each request to determine whether the record(s) are releasable under CCJRA or CORA.
Office Procedure for Handling Records Requests
The length of time needed to respond to requests for criminal justice records governed by Colorado Criminal Justice Records Act (CCJRA) will often take longer and will depend upon the nature of the request. Longmont Public Safety receives a large number of criminal justice records requests each year. Many of these requests require time-consuming review and most require redactions. For these reasons, and depending upon the amount of records requested, criminal justice records requests may take weeks and even months to process.
The requesting party will be notified if the requested records are not available, if the records are not covered by CORA, or they will not be released under CCJRA. A request received after noon or any day the Office is officially closed will be considered received as of the following working day. For CORA requests, the Office may add up to a seven‐working‐day extension if extenuating circumstances apply as described in section 24‐72‐203(3)(b), C.R.S.. The Office will provide all findings of extenuating circumstances to the requestor in writing. Broad, general requests will likely be more costly to the requestor because of the staff time required to fulfill these requests. The Office may contact the requestor in an attempt to clarify or narrow a request. Longmont Public Safety is not required to create reports or compilations of computerized data or to modify existing records in order to create a new record in response to a records request.
Ambulance
The City of Longmont partners with American Medical Response (AMR) for ambulance services. The release of any medical information (including billing) requires a signed Health Insurance Portability and Accountability Act(HIPAA) release.

Fire
Longmont Fire Services is an all hazard response division. The department responds to all fire, medical, hazardous materials, and rescue situations. Office hours are Monday – Friday 8 am to 5 pm.
- To obtain a medical record please complete a Health Insurance Portability and Accountability Act (HIPAA) form and submit with records request. You can call (303) 651-8746 or email to request a report.
- To obtain a fire incident report, you must call (303)651-8502 or email and submit our records request form.
- Be aware that if you are requesting an arson investigative report that there will be two forms required:
- Criminal Justice Request for the police investigation
- Records Request Fire incident

Police
The Longmont Police Records Section is open Monday – Friday, 8 am to 5 pm. Records are available only for incidents that occurred within Longmont city limits. Some reports may be delayed due to ongoing investigations or restrictions under the Colorado Open Records law.
Obtaining an Incident Report
You may request an incident report in one of the following ways:
1. By mail.
Call (303)651-8502 first to confirm that your report is available and releasable. A fee applies to all criminal justice records. Payment can be made by cash, check, or credit card. Credit card payments include a 2.95% processing fee.
Print and complete the English/Spanish request form (pdf), then mail it to: Longmont Department of Public Safety, Records Unit, 225 Kimbark St., Longmont, CO 80501. Do not complete and mail the form WITHOUT contacting records first. Please specify on the form whether you would like the report sent by postal mail or email.
Do not mail the form before contacting Records. Indicate whether you prefer delivery by mail or email.
2. In person.
Staff will verify availability and provide the required form at the Records window (1st floor, Safety and Justice Building, 225 Kimbark St., Longmont, Colorado). You will be informed of the cost at that time.
3. Accident Report Only.
For accidents within the Longmont city limits, you may obtain your report through CARFAX as an alternative to the Records Unit. The CARFAX fee is a flat rate (approximately $5.25). Access to CARFAX.

Obtaining Digital Media
(photos, dispatch tapes, video, body cam footage, etc.):
Digital media can be requested in two ways:
1. By Mail.
Email or call (303)774-4555 to confirm availability or releasability. Fees apply to all digital media. Cash, check, and credit cards are accepted; credit payments include a 2.95% processing fee.
Print and complete the English/Spanish digital media request form (pdf). Complete the form and mail it to Longmont Department of Public Safety, Records Unit, Digital Media Evidence Technician, 225 Kimbark Street, Longmont, CO 80501.
Do not mail the form before contacting the Digital Media Evidence Technician. Specify your preferred method of receiving the media.
2. In person.
Staff will verify availability and provide the required form at the Records Unit Window (1st floor, Safety and Justice Building, 225 Kimbark Street, Longmont, CO 80501). You will be informed of the cost at that time.
